The Buford Family Practice team and Lawrenceville Community Family Center are both made up of board-certified family practice physicians, nurse practitioners, and physician assistants. Buford Family Practice was established in 2006 as a full-service patient care facility in Buford. We have since expanded to Lawrenceville so that we can provide excellent medical services to more patients across Georgia. Both facilities are fully equipped to cater to the healthcare requirements of every family member. Presently, our team comprises Vy Tran Plata, M.D., Annie Howell-Freckleton, FNP-C, and Bayyinah Williams, FNP-C. Our Team is passionate about helping you get healthy and stay healthy. With decades of experience under our belts, we know that providing the best care requires more than medical knowledge, which is why we invest our time into getting to know our patients and your individual health needs, concerns, and goals.
